Tympanoplasty / Ear Drum Surgery in India

Tympanoplasty repairs a perforated or damaged eardrum (tympanic membrane) using a small graft, usually taken from the patient's own tissue (such as the temporalis fascia or tragal cartilage). It is performed to stop recurrent ear infections, restore hearing, and prevent water and debris from entering the middle ear through the hole.

Common causes of eardrum perforation include chronic ear infections, trauma, loud noise exposure, a foreign object injury, or a previous injury that never healed on its own. A persistent hole in the eardrum can lead to recurring discharge, progressive hearing loss, and repeated infections if left untreated, which is why surgeons usually recommend repair once the perforation has been present for more than a few months without healing.

When the middle ear bones (ossicles) have also been damaged by infection or trauma, tympanoplasty is combined with ossiculoplasty, where the surgeon reconstructs or replaces the tiny bones that transmit sound, using either the patient's own bone remnants or a synthetic prosthesis.

Grading by extent of repair

Tympanoplasty is classified into types (I through V) based on how much of the middle ear structure needs reconstruction alongside the eardrum itself. Type I, also called myringoplasty, is the simplest and most common form, involving only closure of the eardrum with the ossicles intact and functioning normally. Types II through V involve progressively more reconstruction of the middle ear bones and structures, depending on how much damage the underlying infection or injury has caused.

How surgeons decide the approach

The choice between a smaller, minimally invasive graft placement and a more extensive reconstruction depends on the size and location of the perforation, the condition of the middle ear bones, and whether there is active infection at the time of surgery. Surgeons typically assess this using a combination of ear microscopy, a hearing test (audiometry), and sometimes a CT scan for complex or recurrent cases before finalising the surgical plan.
