Surgery is the only treatment that offers the possibility of a cure for pancreatic cancer, but it is only an option when the tumor is confined to the pancreas and has not spread to major blood vessels or distant organs. The specific procedure depends on where the tumor sits within the pancreas.

The Whipple procedure

The Whipple procedure (pancreaticoduodenectomy) is used for tumors in the head of the pancreas, the most common location. It removes the head of the pancreas, the duodenum, the gallbladder, part of the bile duct, and sometimes part of the stomach, followed by reconstructing the digestive tract so food, bile, and pancreatic enzymes can flow normally again. It is one of the most complex operations in abdominal surgery, and outcomes are strongly linked to how experienced the surgical team and hospital are.

Distal and total pancreatectomy

For tumors in the body or tail of the pancreas, a distal pancreatectomy removes that portion, often along with the spleen. In some cases, the entire pancreas needs to be removed (total pancreatectomy), which also means the patient will need lifelong insulin and pancreatic enzyme replacement afterward, since the pancreas no longer produces them.

What surgery can and cannot do, and being honest about outcomes

Surgical mortality at experienced high-volume centres is low today, generally under 5%. What matters more for a patient's long-term outlook is whether the tumor could be completely removed and whether it had already spread to lymph nodes at the time of surgery, both of which vary significantly case to case. Pancreatic cancer is a serious diagnosis, and it's important for patients and families to have this conversation honestly with their surgical and oncology team rather than relying on a single statistic. Most patients also need chemotherapy after surgery, which plays a significant role in overall outcome.
